How to Apply for a Contribution for Dental Hygiene

Conditions for Receiving the Contribution:

1. The insured person must be enrolled in the Bonus Plus program at the time the receipt is issued and when the contribution is claimed.
2. Insured persons must provide proof of payment for dental hygiene services with a current date (only receipts from 2024 will be reimbursed in 2024; receipts from 2024 will not be reimbursed in 2025).

What is Not Covered by the Contribution?

• The contribution does not apply to additional payments for health services covered by public health insurance.

How to Apply

1. Via the “Zdraví v mobilu” mobile application (for users of the CPZP Electronic Counter with authorized access).
2. Online through the Electronic Counter on the insurance company’s website.
3. In person at one of the CPZP branches.
4. Alternatively, you can submit the application via data box or by post.

Contribution Payment

1. Payment will be made cashless to the bank account specified on the application form.
