How to Apply for a Contribution for Dental Hygiene

Conditions for Receiving the Contribution:

1. Clients over 18 years of age must have had a preventive check-up with a general practitioner at least once in the last 24 months (this does not include dental check-ups or routine visits to a dentist) before submitting the application.
2. All clients older than 1 year are eligible for the contribution.
3. The insured person must provide proof of payment for dental hygiene services, which must not be older than 3 months.

What is Not Covered by the Contribution?

• The contribution does not cover partial dental hygiene performed by your dentist as part of paid healthcare.
• It cannot be used for procedures, products, or materials during dental treatment.
• The contribution cannot be used to purchase dental hygiene products (such as toothpaste, toothbrushes, mouthwash, dental floss, etc.).
• The insurance company also does not provide a contribution for in-office teeth whitening.

How to Apply

Simply fill out the online form in the Moje VZP application. The insurance company accepts all applications until December 31, 2024.

Contribution Payment

1. Cashless payment to the bank account listed on the application form.
2. Postal order (in exceptional cases)..
