How to Apply for a Contribution for Dental Hygiene

Conditions for Receiving the Contribution:

1. All currently insured individuals are eligible for the contribution.
2. The contribution can only be claimed once per year, though multiple receipts can be submitted within one application.
3. The insured person must provide proof of payment for dental hygiene services, which must not be older than 3 months.
4. Having a preventive examination or other dental procedure with your dentist in 2024 is required.

What is Not Covered by the Contribution?

• The contribution does not cover partial dental hygiene services performed by your dentist as part of paid healthcare.
• The contribution cannot be used to purchase dental hygiene products (such as toothpaste, toothbrushes, mouthwash, dental floss, etc.).

How to Apply

1. You can apply for the contribution online through the VITAKARTA application.
2. The request can also be sent by post or via a data box.
3. Alternatively, you may complete a paper form at OZP service counters.
4. Applications must be submitted by November 30, 2024, when the OZP coupon expires.

Contribution Payment

1. Payment is made cashless to the bank account provided on the application form.
