Conditions for Receiving the Contribution:
- Provide proof of payment for dental hygiene services, which must not be older than 3 months.
- The contribution can only be claimed once.
- Applies to treatments performed by a dental hygienist that are not covered by public health insurance.
What is Not Covered by the Contribution?
- Oral hygiene services performed by your dentist that are covered by health insurance.
- Any procedures, treatments, products, and materials used during a visit to the dentist.
- Purchase of oral hygiene products, such as toothbrushes, interdental brushes, toothpaste, mouthwash, and similar items.
How to Apply
- Submit the application and relevant documents by email, via a data box, through the health insurance company’s web portal (Insured Persons -> Forms for Insured Persons), or by post.
- The insurance company accepts applications for the contribution until November 29, 2024.
Contribution Payment
- Payment will be made cashless to the bank account specified on the application form.
